How often does this turn up?

Diffusion-weighted imaging is part of virtually every brain MRI and of many body MRIs, and it is one of the few sequences whose result can change what happens in the next hour. It does not photograph anatomy; it measures how freely water molecules are jiggling about inside tissue. Where that movement is unusually restricted, the area lights up bright — and that is the sequence doing the one job nothing else does as quickly.

Does it mean something is wrong?

The phrase to look for is not the sequence name but what it found. Restricted diffusion in the brain is the earliest reliable sign of a stroke, visible within minutes when every other sequence still looks normal, which is why this sequence exists at the front of the protocol. It is not exclusive to stroke: dense tumours, abscesses and some other processes also restrict diffusion, and radiologists read it alongside the ADC map to avoid being fooled. Seeing the sequence named in a report says nothing on its own — the sentence describing what it showed is the one that matters.

What to do with it

Nothing follows from the sequence itself; it is a technique, listed under how the scan was done. If it found restricted diffusion, the report says so explicitly and the next steps belong to that finding rather than to the imaging. If it is mentioned only in the technique section with no corresponding finding, the sequence was run and showed nothing — which, on a brain MRI, is a meaningful piece of good news rather than an absence.

What is diffusion-weighted imaging used for?
Above all, finding a stroke early. It detects restricted water movement, which appears within minutes of a blood supply being cut off — far sooner than the anatomical damage becomes visible on other sequences. It is also used to characterise tumours, abscesses and some other tissue.
Is DWI different from a normal MRI sequence?
Yes, in what it measures. Most sequences map tissue properties that translate into an anatomical picture. DWI maps molecular movement, so its images look strange and low-detail on their own and are always read together with the anatomical sequences and the ADC map.
Does my report mentioning DWI mean something was found?
No. Sequence names appear in the technique section as a record of how the scan was performed, whether or not they found anything. What was found is written separately, in the findings and impression.
